THE HEALTH INFORMATION AND EMERGENCY CONTACT NUMBERS THAT YOU PROVIDE AT THE BEGINNING OF THE YEAR ARE VERY IMPORTANT.

Please keep your child’s school informed of any changes in your address or contact numbers, as well as any changes in your child’s health or medical needs. We must have your current contact information at all times, even if your phone number is unlisted.  To update or make changes to your information, please call or go by your child’s school to make changes. 

To better plan for your child’s health care needs during school hours, please answer ALL questions on the health/emergency form.

Based on the information that you provide on the health form; your school nurse may call you to schedule an appointment to further discuss your child’s health needs in order to plan for care during the school day.
